Minnesota Youth in Transition Database (MNYTD) Survey

We want to make sure young people have what they need to transition from foster care into adulthood and live independently, but how do we know what young people need? We ask someone going through it, someone like you! This helps us create better programs and keep funding for young people to successfully transition to adulthood. 

You took the MNYTD survey when you were 17 with your case worker, and the MNYTD Survey Team from The Improve Group (IG) will ask you to take it again around your 19th and 21st birthdays. As a "thank you" for your time, we'll send you a $100 gift card to a store of your choice.

You aren't required to take the survey, and you can skip any question you'd like. 
By taking the survey, you consent to share your answers with IG, Department of Human Services (DHS), and the Children's Bureau. You can request your data from DHS at any time, and your name and contact information will not be shared with anyone else. Click here to learn more about MNYTD.
